- Overview The Elderly Relationship Manager (Nurses) serves a crucial role within healthcare organizations, focusing on the needs and overall well-being of elderly patients. This position is designed for skilled nurses who not only provide medical care but also build supportive relationships with seniors and their families. The ability to offer holistic care is critical, as elderly individuals often face both physical and emotional challenges. By understanding their unique needs, the Elderly Relationship Manager ensures optimal patient outcomes, enhances patient satisfaction, and fosters a safe environment that promotes dignity and respect. This role operates at the intersection of medical assistance and emotional support, requiring a passionate and proactive approach to care. Key Responsibilities Assess, plan, implement, and evaluate individual care plans for elderly patients. Establish and maintain strong relationships with patients and their families. Advocate for patient needs and preferences in care discussions. Monitor vital signs and physical symptoms, reporting changes as necessary. Provide education and resources to families regarding elderly care. Collaborate with interdisciplinary teams to deliver comprehensive care. Develop and facilitate support programs for elderly patients. Coordinate with social services to address additional patient needs. Document patient records accurately and in compliance with regulations. Conduct regular evaluations to monitor the effectiveness of care plans. Assist in planning and executing community outreach initiatives. Participate actively in staff meetings and training sessions. Stay updated on industry advancements and standards of care. Manage any emergencies or crises with appropriate interventions. Enhance patient mobility and daily living skills through tailored programs. Required Qualifications Registered Nurse (RN) license required. Bachelors degree in Nursing or related field preferred. Minimum of 3 years experience in geriatric nursing. Certification in geriatric nursing is a plus. Strong background in patient assessment and care planning. Excellent communication and interpersonal skills. Experience with electronic medical records (EMR) systems. Knowledge of elder care regulations and standards. Ability to work effectively in a team-oriented environment. Demonstrated emotional resilience and crisis intervention skills. Exceptional organizational and time management abilities. Willingness to engage in continued professional development. Skills in cultural competency and sensitivity. CPR and First Aid certification preferred. Understanding of pharmacology and medication management. Familiarity with community health resources available for seniors.
