We are seeking a dedicated and professional Home Healthcare Nurse to join our team. The ideal candidate will provide compassionate nursing care in a home healthcare setting, utilizing all elements of the nursing process, including assessment, planning, implementation, and evaluation. The role demands a strong commitment to patient safety, effective communication, and high standards of documentation and confidentiality.

Key Responsibilities:

• Gather essential information to provide effective care in the assigned home healthcare setting.
• Deliver professional nursing care by leveraging all aspects of the nursing process.
• Assess and evaluate patient status by:
• Developing and initiating a personalized care plan.
• Regularly reassessing patient needs and adjusting the care plan as needed.
• Implement and follow through on physician-prescribed medications and treatment orders.
• Apply preventive, therapeutic, and rehabilitative nursing procedures and techniques while maintaining regular communication on patient condition and care.
• Assist patients with daily living activities as needed.
• Provide nursing services within the scope of expertise and experience.
• Perform specialized procedures, such as chemo port needle insertion, PICC line dressing, catheterization, and Ryle's tube insertion, only when appropriately trained, certified, and authorized.
• Participate in quality improvement programs for nursing practices.
• Comply with patient safety policies and maintain a safe environment.
• Uphold patient confidentiality at all times.
• Adhere to the escalation matrix for patient care needs.
• Maintain a professional appearance, be punctual, and comply with company policies and practices.

Job Requirements:

Minimum Qualifications: -Diploma in GNM or B.Sc. in Nursing

Must be registered with the State Nursing Council.

Experience:- 08 years of post-qualification experience.

Language Skills: - Proficiency in English and Hindi.
