NURSING Wards Role & responsibilities
Nursing ~ OP
Adherence to Nursing Clinical standards:
- Responsible for the OP & Triage routine, equipment and patients.
- Coordinates closely with Guest Relations to plan for appointments; when expected to be busy, to escalate if resources are required.
- Understands patient & consultant requirements and plans the daily activities.
- Ensure the OPD is set for consultation:
- Assist consultants in OPD consult Rooms.
- Sets up procedure trays, dress trolleys and assists doctor in procedures and dressings.
- Assists in transportation of patients whenever necessary.
- Follow strictly all safety measures involved in patient care.
- Assist gynecologists, obstetricians and pediatricians in procedures treating mothers, pediatrics and neonates.
- Ensure all investigations are carried out as advised by the doctor; provides assistance for patients to Lab & Scan rooms.
- Handles emergencies competently & calmly; follows instructions competently.
- Ensures In-charge & NS is informed of any incidents immediately.
- Ensures medication safety and practices patient safety protocols.
- Upholds patient rights and maintains confidentiality of patient information: medical and personal; avoids any violation by ensuring no discussion of patient identifiable information in non-secure areas.
- Provides health education and counseling to patient and attendants.
- Implements the infection control practices.
- Implements NABH & Nursing Excellence standards.
- Competently handles all equipment used in patient care; ensures team is competent in usage; ensures equipment is serviceable and readily available.
- Co-ordinates with Billing team regarding investigations and billing activities.
- Works closely with clinical services team to ensure efficient care.
- Follows MTP, PCPNDT rules and regulations
NURSING Wards
Adherence to Nursing Clinical standards - Renders professional bedside nursing care.
- Prepares nursing care plans for assigned patients with an understanding of individualizing treatment plans, patient compliance, acceptability and comfort.
- Attends call bell within 2 minutes.
- Functions as SPOC for all non-medical services, to ensure speedy service.
- Conducts Room orientation competently for all admissions.
- Completes the Nursing Assessment Guide within 30 minutes of patients admission.
- Reviews the doctors notes and drug chart on admission and implements it.
- Checks and records vital signs legibly and promptly.
- Accompanies Consultants on rounds; carries out orders completely.
- Ensures that all investigations are conducted as advised by the doctor, reports available on time for doctors perusal and further advice.
- Assist the doctors in special procedures, dressings and rounds.
For following procedures , ensures procedure trolleys are correctly set:
- Coordinates procurement of blood and blood products for transfusion. Any reactions must be reported to treating consultant, IC & NS immediately.
- Accompanies patient for special procedures and investigations.
- Prepares the patient for labor room and delivery.
- Completes the transfer formalities as per the In-house Transfer Policy.
- Completes the preoperative checklist before sending the patient to operation theatre.
- The Obstetrics & Gynecology nurse has specific duties in providing care for mothers and their babies during pregnancy - helps expectant parents prepare for labor and delivery.
Offers health education and counseling in:
NURSING Operation Theatre
Adherence to Nursing Clinical standards:
- Understands the OT routine, equipment and patients.
- Aware of the O.T. protocols and the layout.
- Checks the surgery schedule and prepares for the cases: medication, consumables, instruments and equipment are available for the surgery.
- Checks all equipment are functional; conducts random checks of surgical and laparoscopic instruments ensuring they are functional.
- Identifies instruments as per the surgery, checks cleanliness and proper functioning of instruments, pack and send for autoclaving / gas sterilization.
- Strictly follows aseptic technique.
- Is aware of and implements all protocols - hand washing, infection control, medication, narcotic policy, personal valuables, follows the dress code in and out of the restricted areas.
- Assists in surgical procedures.
- Receives the patient in the preoperative area, checks for: right patient, correct documents, right surgery and transfers to the Operation Theatre.
- Checks the preparation of the patient for cleanliness, skin shaving for the particular surgery.
- Follows pre-anesthetic medications if any.
- To check and record vital signs legibly and promptly.
- Ensures that all investigations are carried out as advised by the doctor, reports available on time for doctors perusal and further advice.
- To ensure that all the materials needed for the surgeries as per Procurement Monitoring Report sheet and instrument sets, Linen, Gowns, etc., are available.
- Procures all blood and blood products for transfusion. Any reactions must be reported to
treating consultant, IC & NS immediately.
- Strictly follows WHO surgical safety process in OT count of instruments, consumables - to check the instrument trolley carefully and record items likely to be left inside.
- Checks for the right patient, I.D. No., site of surgery, type of surgery, and operating surgeon when the anesthetist calls, Time out inside the operation theatre.
- Rechecks instrument trolley items as per record and gives clearance to the surgeon for closing.
- Any errors in medication / nursing intervention to be brought immediately to the notice of OT In-Charge.
- Monitors Histopathology sample dispatch as per standards.
- Handles waste segregation and disposal as per the policy.
Nursing ~ NICU
Adherence to Nursing Clinical standards:
- Oriented to the NICU routine, equipment and type of neonates.
- Outstation Neonate Retrieval by Ambulance: provide nursing care through the retrieval, accompanies and assists.
- Complete the Nursing Assessment Guide within 15 minutes of neonate admission.
- Ensures the neonate is comfortable, implements safety measures for vulnerable neonates as per the policy.
- Ensures safety and security of neonates by following safety measures.
- Neonate condition updates to be shared twice a day with parents; change in condition to be informed immediately; provide comfort and emotional strength to parents.
- Review the doctors notes and the drug chart on admission and implement action as required.
- Prepares nursing care plans and render professional bedside nursing care.
- Ensure investigation slips are raised.
- Ensure all investigations are carried out as advised by the doctor, reports available on time for doctors perusal and further advice.
- Raise pharmacy indents, check on receipt, and stock in the bedside locker and medication trolley.
- Specific responsibility towards neonate physical needs:
- Feeding
- Position
- Medication dosage and calculation according to weight and body surface
- Monitor neonates on incubator and ventilator and inform abnormal findings.
- Responsible for checking and recording daily weight.
- Responsible for monitoring and maintaining normal oxygen saturation level.
- Administer on time medications and therapies.
- Check and record vital signs legibly and promptly.
- Assists the doctors in special procedures, dressings and rounds.
For the following procedures, ensure properly set procedure trolleys.
