Job Description

Role & responsibilities

• Patient Assessment: Regularly assess patients' physical and emotional well-being, including vital signs and wound care.
• Medication Administration: Administer prescribed medications, ensuring proper dosage and timing.
• Wound Care: Provide wound care services, including dressing changes and monitoring for healing.
• Education: Educate patients and families about their condition, medications, and safety measures.
• Care Planning: Develop and implement individualized nursing care plans based on patient needs.
• Documentation: Maintain accurate records of patient assessments, interventions, and progress.
• Communication: Communicate with physicians and other healthcare providers to coordinate care.
• Personal Care: Provide personal care services like bathing and feeding, if needed.
• Collaboration: Work collaboratively with other healthcare professionals, such as physical therapists and social workers.
• Safety: Ensure a safe environment for patients in their homes.

Additional Responsibilities:

• Monitoring: Monitor patient's condition and progress, reporting any changes to the physician.
• Referral: Refer patients to other healthcare professionals or resources as needed.
• Equipment Management: Manage durable medical equipment used in patient care.
• Telemonitoring: Utilize technology for patient monitoring and communication (telemonitoring or telehealth).
