Job Description

  • Client Assessment:
  • Conduct comprehensive assessments of clients needs, preferences, and goals.
  • Work closely with clients to develop personalized support plans.
  • Service Coordination:
  • Identify and connect clients with appropriate support services, resources, and programs.
  • Collaborate with service providers to ensure the delivery of high-quality and tailored support services.
  • Documentation:
  • Maintain accurate and up-to-date client records and documentation.
  • Prepare reports on client progress and support plan outcomes.
  • Communication:
  • Communicate effectively with clients.
  • Facilitate regular meetings to review and adjust support plans as needed.
  • Only Immediate joiners or candidates with a lesser notice period of 15 days are required.
