Job Description
- Community Assessment and Health Promotion: PHNs assess the health needs of communities, identify at-risk populations, and develop targeted interventions.
- Disease Prevention and Control: They play a crucial role in implementing and evaluating public health programs, conducting screenings, and administering immunizations.
- Health Education and Advocacy: PHNs educate individuals and groups on health promotion, disease prevention, and healthy lifestyle choices. They also advocate for policies that support community health.
- Community Outreach and Collaboration: PHNs work with community stakeholders, including schools, community organizations, and government agencies, to deliver health services and promote access to care.
- Emergency Response: They may be involved in responding to public health emergencies, such as disease outbreaks.
- Monitoring and Evaluation: PHNs track health trends, collect and analyze data, and evaluate the effectiveness of public health programs.
- Addressing Health Disparities: They work to identify and address social determinants of health that contribute to health inequities within communities.
